Geriatric Nurse Practitioner

Responsibilities
- Conduct comprehensive geriatric assessments, including physical examinations, cognitive assessments, and health history reviews.
- Develop and implement personalized care plans for elderly patients, focusing on their unique needs and preferences.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate and optimize patient care.
- Prescribe medications and treatments, monitor their effectiveness, and make adjustments as necessary.
- Educate patients and their families on managing chronic conditions, preventing falls, and promoting healthy aging.
- Provide counseling and emotional support to patients and their families regarding end-of-life care and advanced care planning.
- Conduct regular follow-up visits to assess treatment outcomes, monitor patient progress, and address any concerns or issues.
Requirements
- Master's degree in Nursing and certification as a Nurse Practitioner.
- A valid state license as a Nurse Practitioner.
- Minimum of 2 years of experience working as a Nurse Practitioner, preferably in geriatric care.
- In-depth knowledge of geriatric assessment tools and evidence-based geriatric care practices.
- Strong clinical judgment and decision-making skills.
- Excellent communication and interpersonal skills, with the ability to build rapport with elderly patients and their families.
- Ability to work collaboratively in a multidisciplinary team.
