Physical Therapist

Coker Rehabilitation Services, Inc (www.cokerrehab.com), an established contract therapy company in business for over 20 years, is seeking a prn PT for adult home health in Corinth/Denton/Flower Mound who will provide direct patient care to homebound patients.

• $80 per visit contract/1099 or $160,000 per year contract/1099
• Set your own flexible schedule and hours
• Therapist mentorship
• Work as little or as much as you want

Responsibilities:

- Providing quality physical therapy evaluation and treatment intervention in patient homes as outlined in the plan of care and in accordance with physician orders/protocols, patient's level of function, Medicare and state licensing guidelines, and home health agency policies and procedures.
- Provide continuity of care to include consulting with supervising MD regarding patient progress or lack thereof, and reporting patient concerns to agency, including any changes in patient's condition.

Qualifications include the following:

- Current Texas License.
- Current Certification in CPR
- Valid Texas Drivers License and Insurance, with reliable transportation
